The majority of PEI's health burden is caused by four chronic diseases:
cancer, cardiovascular disease, chronic pulmonary disease, and diabetes,
which are strongly linked to poor diet, lack of physical activity, tobacco use
and excessive alcohol consumption.
PEI's population has poorer health outcomes than the rest of Canada.12
PEI is engaging in monitoring and tracking for these illness at home through
initiatives such as Remote Monitoring, which helps with self-management
and lessens admission rates to hospitals. 41,42

Improve the disease prevention and management and ease the public
access of the available services.
• Tackle systemic issues that prevent good chronic disease management,
provide the supports for patients to properly manage their disease at home.
Increase collaboration of healthcare professionals across the continuum of
