Target
The 2022-23 target for the percentage of people accessing community-based
palliative care who opted to die at home, and who were supported to die at home,
is 76 per cent or above.
Improved or maintained performance is demonstrated by a result equal to or
above the target.
Results
In 2022-23, 73 per cent of clients requiring end-of-life care who opted to die
at home were supported to do so through the provision of in-home specialist
palliative care (see Figure 12). This result is slightly below the specified target of
76 per cent.

Notes:
1. There is currently no national target for this indicator. The 2022-23 target is based on an average of prior year results.
2. Specialist community-based palliative care refers to palliative care that is provided by a multidisciplinary team within
a private residence (i.e. 'in-home' care), but not a residential care facility.
3. The calculation of this indicator is based on:
a. People living in the Perth metropolitan area who have an active, progressive and advanced disease, who require
access to specialist palliative care services.
b. Access to services where a medical opinion has been obtained resulting in the client being referred for specialist
palliative care.
c. People who accessed the community palliative care service provided by Silver Chain and who died at home after
nominating this as their desired place of death.
4. There are some community-based palliative care services provided outside of the metropolitan area; however,
this activity data is not available within the Non-Admitted Patient Data Collection system used to source data for
calculation of this KPI.
