small group employment support for individuals with ID/DD ages 0 - no max
age.
Provides respite, service management, and assistive technology for individuals
with autism ages 0-15yrs.
Medically
Fragile
Provides institutional respite, program management or case management,
dietary supplements, environmental modifications, equipment and supplies, in-
home supports, individual and family counseling, and transportation for
individuals who are medically fragile ages 3-17.
