Emergency department alternative care pathways
The department allocated $19.2 million in funding to support health
service providers to trial new initiatives aimed to relieve pressures
on emergency departments. These innovative pilots aimed to drive
improvements across the health system and to identify opportunities for
further reform.
The initiatives focused on supporting a range of patient cohorts (older
adults, and clients with mental health, oncology and musculoskeletal
conditions) and an initiative that targeted clinical emergency department
documentation processes. Initiatives were implemented across
metropolitan and country health service sites.
Common to all initiatives is the provision of an alternate pathway to
emergency department attendance and or admittance. These new
models of care aim to reduce emergency care demand by diverting
patients to receive the right care in the right place, supported by the right
resources, and in doing so further reducing the risk of re-presentations
to emergency centres.
An estimated 11,189 patients have been engaged through these
initiatives in the latter part of 2022 with results for the targeted patient
cohorts showing reductions in emergency department presentations
and re-presentations, unplanned hospital admissions, and average wait
times for referred patients (See Table 2).
