GOALS, OBJECTIVES, STRATEGIES & MEASURES GOAL 3: ELDER JUSTICE Promote systems of protection and safety that facilitate dignity and respect LTC OMBUDSMAN Objective Strategies Improve the quality of LTSS by offering high quality, person-centered advo- cacy to individuals as well as providing systemic LTSS advocacy Measures · • • • · • Provide technical assistance on the LTC Ombudsman Program to local ombudsman entities and ombudsman representatives Integrate federal LTC Ombudsman regulatory requirements into the state LTC Ombudsman regulations Respond to inquiries from and provide assistance to individuals en- rolled in Medicaid CCC Plus who need beneficiary support services and assistance resolving problems Transition to the 2019 ACL National Ombudsman Reporting System data standards to allow for improved data collection and program monitoring Conduct annual trainings for ombudsman representatives Serve as a lead partner in LTC culture change efforts and the Virginia Culture Change Coalition Monitor LTSS and Medicaid managed care policy changes at the feder- al and state level and provide feedback as appropriate and needed Engage with LTC and HCBS providers to improve the quality of LTSS Advocate for increased LTSS staff training on person-centered practic- es, mental health conditions, and co-occurring diagnoses, such as de- mentias, mental health, and substance use disorders • Successful revision to state regulations to ensure alignment with Code of Virginia and federal regulations for the LTC Ombudsman Program by 2022 Number of CCC Plus beneficiaries assisted - annually • Number of LTC complaints investigated and resolved - annually 68
